چکیده

Herein we describe a case of a 56-years old patient presented with general malaise, anorexia and progressive weight loss for about 5years associated recently with diarrhoea and repeated syncopal attacks for 1month. On admission, he had splenomegaly and his laboratory studies showed macrocytic hyperchromic haemolytic anemia [Hb = 9.4g/dl] and a signifi cant hyponatremia [Na+ = 111mmol/l]. Endocrinological evaluation revealed an isolated adrenocorticotropin defi ciency (IAD) with a manifest hypoaldosteronism. On treatment with cortisone, both anemia and hyponatremia (but not splenomegaly) were markedly improved. Our case may represent an evidence for the pathogenesis of erythropoietic dysfunction in patients of adrenocortical insuffi ciency.

Possible Mechanisms for The Effects of Calcium Defi-ciency on Male Infertility

Calcium (Ca) is a significant element that acts as an intracellular second messenger. It is necessary for many physiological processes in spermatozoa including spermatogenesis, sperm motility, capacitation, acrosome reaction and fertilization. Although influences of Calcium deficiency on sperm function and male infertility have been widely studied, mechanisms for these abnormalities are not wel...

Congenital Diaphragmatic Hernia and Iron Deficiency Anemia

The case described by Dr. Sarper and her colleagues in their letter to the editor, entitled “Severe iron defi ciency anemia due to late presentation of congenital diaphragmatic hernia in a toddler”, was not a late but rather a delayed diagnosis case since iron defi ciency anemia was diagnosed in this patient at least a year earlier with intermittent vomiting [1]. The diagnosis of a diaphragmati...

Severe lactic acidosis reversed by thiamine within 24 hours

iamine is a water-soluble vitamin that plays a pivotal role in carbohydrate metabolism. In acute defi ciency, pyruvate accumulates and is metabolized to lactate, and chronic defi ciency may cause polyneuropathy and Wernicke encephalopathy. Classic symptoms include mental status change, ophthalmoplegia, and ataxia but are present in only a few patients [1]. Critically ill patients are prone to t...
